Insurance Waivers #
Waiver Requirements Notice
Annual Membership Waiver Forms: All Active members of USA-WSWS are required to sign an Annual Membership Waiver as a part of the membership process. Current Active members who do not have a signed Annual Membership waiver on file will be prompted to sign their waiver electronically when logging into their Member Dashboard.
There are two versions of the Annual Membership Waiver. An Adult version for Active members Age 18 or older, and a Minor version for Active members under Age 18. The Minor waiver form must be signed by a parent/legal guardian.
Specific Event Waiver Form: Each participant in a USA-WSWS sanctioned event must sign a Specific Event Waiver form as part of the registration process for the event.
There are two versions of the Specific Event Waiver. An Adult version for participants Age 18 or older, and a Minor version for participants under Age 18. The Minor waiver form must be signed by a parent/legal guardian.
Active members who have signed the Annual Membership Waiver form are not required to sign a Specific Event Waiver for sanctioned practices.
Guest members must sign a Specific Event Waiver Form at each practice.
Note to Registrar: #
The Participant Waiver forms are no longer a part of the Tournament Entry Forms. It is the Local Organizing Club’s (LOC’s) responsibility to retain possession of all Specific Event waivers. The Adult waiver forms should be kept for a minimum of 6 years depending upon the state, while the Minor waiver forms need to be kept until the minor reaches the age of 24 in some states (Age 18 plus 6 years). The statute of limitations does not begin until minors reach age 18. The waiver forms must be able to be produced by the LOC at the time a claim is brought.
